Application Process

What is required at the time of application
- Student's most recent report card
- Most recent standardized test scores (grades 2-8)
- Birth certificate
- Baptismal & Communion certificates (if applicable)
- Copy of 504 Plan (if applicable)
- State required health records form #2040/680 prior to first day
Paperwork and all forms can be uploaded during the application process. If you are unable to upload, all paperwork and forms may be turned in at the main office.How to Apply
To begin the Online Application process, Create an Account. Use school code: CCCS-Fl .Upon account creation, you will receive an email containing a verification link. Once your email address has been verified, log in to your account and Create A New Student Application for your child. You will then have the flexibility to log in and out of your account and access your open application.

- New applications are reviewed once all forms have been submitted and the admissions assessment/test has been completed. Once your application is submitted, you will be advised of the testing date. Applications for 2024-25 will open February 5th, 2024.
Please be advised that class sizes are limited and waiting lists are likely. We are no longer accepting applications for grades 2,3, 5 & 6 for the 24-25 school year.
